Care Plans

Patients are eligible to receive Medicare reimbursement for up to 5 reviews per 12 month period by a Dietitian, Diabetic Educator or Exercise Physiologist if referred under an EPC plan that their GP has prepared and billed the following MBS Chronic Disease Management items in the previous two years:

• A GP Management Plan – item 721 (or review item 725); and
• Team Care Arrangements – item 723 (or review item 727)

Medicare reimbursement for Psychology services can only be provided to people with ‘an assessed mental disorder’ that is being managed by a GP under a GP Mental Health Care Plan or a psychiatrist assessment and management plan, or on direct referral from a psychiatrist or a paediatrician (for treatment of a child).

All psychology Medicare services are limited to a maximum of 5 individual sessions per client per calendar year.
